COVID-19 Impact on the global defibrillator market

The COVID-19 pandemic adversely affected the economy in 2020 and presented various challenges to clinical healthcare providers and patients across the globe. Logistical issues, managing patients with the disease, prioritizing patients with comorbidities & pre-existing conditions, and further protecting the public & hospital frontline workers from COVID-19 are major challenges healthcare systems face across the globe.

The pandemic has also impacted the buying capacity of hospitals, especially small-scale hospitals, resulting in canceled or postponed supply deals, which has impacted the overall revenue of all the major players in the defibrillators market. Also, due to lockdowns, many people lost their jobs and livelihood, which, in turn, affected the buying pattern of patients considerably.

Arrhythmias were observed in many COVID patients, but in-hospital defibrillator facilities helped save their lives. Due to the virus outbreak, many surgical procedures were canceled or postponed, depending on the criticality of the patient. This resulted in losses for companies.

COVID-19 also impacted various stages of the value chain, primarily due to strict lockdowns imposed by the governments of most countries. The turnaround time for the delivery of products and services was also affected due to newer packaging protocols and lockdown measures imposed by countries. All these factors negatively impacted the manufacturing and supply chain of the defibrillators market.

Defibrillator Market Dynamics

Driver: Rising prevalence of target disease

Cardiovascular disease (CVD) is a leading cause of death and disability across the globe. Globally, 17.9 million deaths due to CVD were reported in 2019, which amounts to 32% of the total deaths worldwide (Source: WHO). More than 75% of deaths due to CVDs occur in low- and middle-income countries. In the US alone, there were more than 390,000 sudden cardiac arrests in 2018 (Source: American Heart Association). Of the total US population, around 65% of deaths due to CVDs occur in people aged 75 and over, 29% in the age group of 55–74, and 6% in the age group of 35–54. Owing to this, the demand for defibrillators has increased, as these devices help save the lives of patients at a high risk of cardiac arrest.

The prevalence of lifestyle diseases such as hypertension, obesity, dyslipidemia, and diabetes, which are major risk factors for CVD, is also on the rise. There is a strong linear relationship between high blood pressure levels and the risk of developing CVDs. In the US, around 70% of people who have suffered from their first cardiovascular stroke, their first heart attack, and who have congestive heart failure, are reported to have a blood pressure of more than 140/90 mm Hg (Source: American Heart Association).

In Asian countries, over the last decade, the prevalence of CVD has increased due to the rising adoption of sedentary lifestyles, changing diets, increasing stress, and rising consumption of alcohol and tobacco. A study on heart disease conditions in Asia stated that the prevalence of heart diseases is expected to increase by over 110% between 2000 and 2020. Furthermore, CVD death rates in India are estimated to have risen from 155.7 to 209.1 per 100,000 between 1990 and 2016 (Source: Journal of the American College of Cardiology).

The rising geriatric population across the globe is another key factor contributing to the increasing prevalence of CVD, as aging is associated with progressive deterioration in the structure and function of the heart and acts as a catalyst in the development of chronic diseases. Thus, with the growth in the geriatric population across the globe, the incidence of heart diseases is expected to increase.

Restraint: Issues related to the use of implantable and automated external defibrillators

AEDs installed in public places are sometimes prone to failure due to problems pertaining to the battery, software, and hardware. Failure to perform defibrillation can result in delayed treatment or even death. Over the years, a number of defibrillators have been recalled due to technical errors in the functioning of the device with respect to shock delivery, electrical issues, and hardware configuration. These defects may lead to undesirable product performance and even the death of the patient in some cases. In 2018, the FDA, in conjunction with Philips, issued a recall notice for various models of the HeartStart MRx Defibrillator. The recall was caused by a defect in the gas discharge tube (GDT), which could lead to the malfunction of the defibrillator. In 2021, Medtronic recalled the Evera, Viva, Brava, Claria, Amplia, Compia, and Visa ICDs and CRT-Ds due to the short battery life associated with these devices.

Cybersecurity is another major concern in the defibrillators market. The new generation of implantable defibrillators is technologically advanced, allowing for remote data transfer. The FDA has raised concerns regarding the hacking of these devices. Medtronic was the latest company to face an FDA cybersecurity issue in 2019, as Medtronic’s wireless telemetry technology did not have encryption or authentication safety.

ICD leads also have a high rate of electrical failure. Around 4.5% of the 4,000-plus active Riata ICD leads had an electrical failure five years after implantation in Canada. Electrical failures are mainly caused due to the breakdown of insulation around the wires. Lead failures are associated with loss of device function, inappropriate shocks, and sometimes death. In 2020, Boston Scientific recalled its EMBLEM S-ICD due to a short circuit in the device.

Other major issues related to AEDs include faults in the defibrillation pads, wiring and circuit boards, and damaged cables and connectors. In some cases, the batteries of AEDs are removed when they are discharged; however, these batteries are not replaced on time. Although a number of initiatives are taken by public and private organizations as well as key market players to provide emergency services, these devices require proper maintenance and periodic servicing.

Opportunity: Emerging Markets

Emerging markets offer significant growth opportunities for players in the defibrillators market, owing to the rising incidence of cardiovascular disease and improving healthcare infrastructure in these countries. According to a research study published by the American Heart Association, annual cardiovascular events in China are projected to increase by more than 50% from 2010 to 2030. Similarly, cardiac ailments killed approximately 28% of Indians in 2016, double the numbers reported in 1990 when heart diseases caused 15% of the total number of deaths.

To leverage the high growth opportunities for defibrillator devices in emerging markets, manufacturers are strategically focusing on expanding their presence in these countries. For instance, in May 2016, Medtronic opened its Asia Pacific headquarters in Singapore to support operations at 80 locations in this region. In January 2016, Nihon Kohden Corporation established its sales subsidiary, Nihon Kohden Mexico S.A. de C.V., in Mexico to provide sales and aftersales support for the company’s products and services.

Regulatory policies in the Asia Pacific region are more adaptive and business-friendly due to fewer data requirements than in developed economies. Increasing competition in mature markets will further compel defibrillator manufacturers to focus on emerging markets in the coming years.

Challenge: Frequent product recalls

In recent years, the market has witnessed a number of product recalls mainly due to reasons related to electrical failure, connector compatibility issues, and issues pertaining to specific defibrillators. Between January 2005 and September 2014, the FDA received 72,000 medical device reports related to AED failure. In the same period, manufacturers of AEDs conducted 111 product recalls, affecting over 2 million AED units. During 2016–2019, the FDA issued around 70 product recalls for defibrillators/monitors, AEDs, ICDs, CRT-Ds, and accessories. Frequent product recalls pose a major challenge to the defibrillators market.

However, strategic initiatives undertaken by manufacturers, such as the launch of improvement programs and the development of better-performance defibrillators, help address device-related complaints.

Defibrillators Market Ecosystem

The Implantable Cardioverter Defibrillator segment accounted for the largest share of the global defibrillator market, by product type, in 2020

Based on product type, the defibrillator market is segmented into implantable cardioverter defibrillators and external defibrillators. The implantable cardioverter defibrillator (ICDs) segment is expected to dominate this market during the forecast period. The large share of this market segment can be attributed to the increasing prevalence of cardiovascular diseases and the development of technologically advanced defibrillators

The adult patient segment, by patients type, accounted for the largest share of the global defibrillator market in 2020

Based on patient types, the defibrillator market is segmented into adult  and pediatric. The adult patient segment segment accounted for the largest market share in 2020. The adult patients segment dominated the global defibrillators market with a share of 59.5% in 2020.

In 2020, there were roughly 75 million adult patients (in the age group of 15 years and over) globally and 25 million pediatric patients (in the age group of 0–14 years) (Source: World Bank). According to data by the World Health Organization (WHO), around 30% of all global deaths occur due to cardiovascular disease, accounting for 17.9 million lives lost each year. About 2 in 10 deaths due to CVD (cardiovascular disease) occur in patients aged less than 65 (Source: Center for Disease Control and Prevention). The major reasons for heart disease are high blood pressure, high cholesterol, diabetes, obesity, unhealthy diets, physical inactivity, and excessive alcohol use.

The development of advanced ICDs, CRT-Ds, and AEDs, along with increasing awareness about cardiac diseases and preventive measures taken by government bodies, are the major factors contributing to the growth of the market.

On the basis of End Users, hospitals, clinics and cardiac centres segment accounted for the largest share of the global defibrillator market, in 2020. While Public access markets is expected to grow with the highest growth rate during the forecast period of 2021-2026.

Based on end users, the defibrillators market is segmented into hospitals, clinics, and cardiac centers; prehospital care settings; public-access markets; alternate care facilities; home care settings; and other end users. The hospitals, clinics, and cardiac centers segment dominated the global defibrillators market, with a share of 80.2%, in 2020.

A majority of all implantable cardioverter defibrillator (ICD) procedures are carried out in hospitals, clinics, and cardiac centers. Defibrillators are typically placed in critical care units (CCUs), intensive care units (ICUs), and emergency units. ICDs and external defibrillators (manual as well as AEDs) are the most common types of defibrillators used in these facilities. Although manual defibrillators are preferred and are extensively used in hospitals, the use of AEDs in hospital settings is on the rise. Early defibrillation, shorter response time, ease of use, cost-effectiveness, and a shortage of trained staff have increased the preference for AEDs in these facilities.

The growing number of cardiac implant procedures and the availability of reimbursements across developed markets are the other major factors driving the growth of this end-user segment. According to the American Heart Association, an estimated 10,000 ICD procedures are carried out each month in the US alone (data as of July 2016). Similarly, the total number of ICD implants in England increased to 72 per million population in 2014 from 66 per million population in 2012 (Source: British Heart Rhythm Society).

The development of advanced ICDs and CRT-Ds and the large number of ICD and CRT-D implant procedures performed in hospitals, clinics, and cardiac centers are the major factors contributing to the large share of this end-user segment.

The public-access markets segment includes trained and untrained personnel and bystanders who specifically use AED devices to treat sudden out-of-hospital cardiac arrests. The US and certain European and Asian countries have passed legislation regarding installing AEDs in public places and the use of AEDs by bystanders. As a result, AEDs have been installed in public places such as community centers, schools, stadiums, health clubs, recreation centers, shopping places, transportation centers, government buildings, airlines, and offices.

In 2020, the public-access markets segment accounted for a share of 2.7% of the defibrillators market. This segment is expected to register the highest growth of 11.3% rate during the forecast period. The rising prevalence of sudden cardiac arrests (SCAs) is a major factor supporting the growth of this end-user segment.

Defibrillators Market by Region

The Asia Pacific market is expected to grow at the highest CAGR during the forecast period

On the basis of region, the defibrillator market is segmented into North America, Europe, Asia Pacific, Latin America, and the Middle East & Africa. The Asia Pacific region is expected to register the highest growth during the forecast period. The Asia Pacific is segmented into China, India, Japan, South Korea, Australia, and the Rest of Asia Pacific, including Malaysia, Taiwan, Singapore, and New Zealand.

Growth in this market can be attributed to the rapidly growing geriatric population and the subsequent increase in the prevalence of CVDs.
