COVID-19 Impact on the Oral Care/Oral Hygiene Market

In 2020, the COVID-19 pandemic significantly impacted distribution channels and consumer behavior related to essential and non-essential goods, with similar trends seen in the oral hygiene and oral care market. In the first quarter (Q1) of 2020, as the COVID-19 pandemic started to accelerate, the initial trend of stockpiling mainly benefitted consumer stores as most customers were bulk purchasing from consumer stores before lockdown. However, according to the US Census Bureau of the Department of Commerce, overall retail sales in Q1 of 2020, of which consumer store sales form a major part, showed a 1.3% decrease from Q4 (last quarter) of 2019.

The most prominent impact of the COVID-19 imposed lockdown, social distancing, and quarantine measures have been the shift in consumer purchasing patterns from in-store to online. According to the US Census Bureau of the Department of Commerce, in the first quarter of 2020, US retail e-commerce sales increased by 14.5% compared to the first quarter of 2019. In contrast, total retail sales increased by only 2.8% in the same period. This trend can be expected to reflect in the oral care and oral hygiene market, as consumers will favor contactless purchasing even in the post lockdown era.

Sales of oral care products triggered due to visits to dental clinics/dental practices are expected to return to the pre-pandemic scenario by Q1 – Q2 2022.

Oral Care Market Dynamics

Driver: Rising Incidence of dental diseases

There has been exponential growth in the global burden of various dental diseases, which would increase the adoption of various oral care products. Tooth decay (dental caries) and periodontal diseases (gum diseases) are the most widespread chronic diseases worldwide and constitute a major global public health challenge. The treatment of dental caries is expensive, accounting for around 5–10% of the total expenditure on healthcare borne by developed nations (Source: WHO). A study conducted by the Centers for Disease Control and Prevention (CDC) in 2018 revealed that from 2015-2018 the total percentage of untreated dental caries was 13.2% (for ages between 5 and 14) and 25.9% (for ages between 20-44 years).

According to the FDI World Dental Federation, the prevalence of dental caries is significantly higher in the younger population. The prevalence of dental caries in the 6-19 age group is as high as 60% to 80% in developed countries across North America and Europe. In contrast, in developing countries, including India, Brazil, and Mexico, its prevalence is more than 80%. The high incidence of dental caries and other periodontal diseases is key in driving demand for oral care products.

Restraint: Product recalls

The incidence of product restrains the growth of the oral care market recalls in recent years. For instance, in July 2018, Water Pik Inc. (US) voluntarily recalled its Sonic-Fusion flossing toothbrush due to consumer reports of the product malfunctioning in the US. Reports indicated that the charging base of the toothbrush was overheating with localized melting and sparking, causing a risk of shock or burns. In October 2020, Sunstar America Inc. (SAI) (US) recalled the company’s Paroex Chlorhexidine Gluconate Oral Rinse 0.12% (expiration between June─September 2022) product due to possible contamination with Burkholderia lata. These risk factors could deter consumers from purchasing ancillary oral hygiene products such as mouth rinses/mouthwashes and could, in turn, restrict the growth of the overall market.

Opportunity: Rising trend of online purchasing and e-commerce

There has been a marked shift in distribution channel trends for oral care products in recent years. Online retailers are gradually being preferred over traditional distributors, retailers, and pharmacies. These online retailers provide a wide range of oral care products at attractive prices. The growing trend of online purchasing is supported by the increasing number of Internet users, which has reached 4.8 billion (as of July 2021) globally (Source: Kepios). According to Miniwatts Marketing Group, as of June 2019, oral care products accounted for 13% of the personal care products ordered by online portals. Moreover, purchasing oral care products through e-commerce portals offers several advantages, such as the availability of multi-brand oral care products at relatively lower prices, ease of purchasing, and free shipping.

The COVID-19 scenario has encouraged the online distribution of oral care products considerably. Due to the lockdown and quarantine measures undertaken during the pandemic scenario, consumers have increasingly shifted to e-commerce and online channels to purchase oral care products, thus further resulting in the growth of the online mode of distribution in the past year.

Challenge: Adverse effects of teeth whitening products

Although a growing trend in the oral care market, teeth whitening also has the potential to cause certain adverse effects in consumers. Repeated use of whitening products such as teeth whitening pens can cause irreversible destruction of the tooth enamel. This could, in turn, cause teeth sensitivity due to repeated erosion of the protective enamel layer. Also, the teeth whitening agent is likely to contact the gum line during use, which can cause localized irritation and pain. Whitening agents can also cause uneven teeth whitening, especially if a few or several of the teeth in the dentition are artificial.

Moreover, these products should not be used when there are open cavities, causing leaching of the product to the roots, thereby causing further tooth damage. These adverse effects could deter the use of these products and thereby restrain the growth of the cosmetic dental whitening products segment in the oral care market. However, certain peroxide-free alternatives for teeth whitening currently available and under development may allow the market to overcome this challenge. For instance, in recent studies, it has been seen that cysteine protease enzymes, such as papain and bromelain, have been described as active agents with bleaching potential.

Based on product, the oral care market is categorized into toothpastes, toothbrushes & accessories, mouthwashes/rinses, denture products, dental accessories/ancillaries, and dental prosthesis cleaning solutions.

The toothpastes segment accounted for the largest share of the oral care market in 2020. The toothbrushes & accessories segment is expected to register the highest growth rate during the forecast period (2021-2026). The high growth of this segment is mainly attributed to the increasing adoption of electric toothbrushes and replacement toothbrush heads, especially in developed markets.

The oral care market is segmented into consumer stores, retail pharmacies, online distribution, and dental dispensaries based on distribution channels. In 2020, the consumer stores segment accounted for the largest share of the oral care market. The large share of the consumer stores segment can be attributed to the many stores worldwide and easy accessibility.

Asia Pacific holds the largest share in the oral care market, by region, in the forecast period

The oral care/oral hygiene market is segmented into North America, Europe, Asia Pacific, Latin America and Middle East & Africa. Asia Pacific accounted for the largest share in the oral care/oral hygiene market in 2020. The growing dental tourism in major Asian markets (such as India, South Korea, and China); the increasing focus of prominent players on emerging Asian countries; increasing healthcare expenditure (coupled with the rising disposable income); a growing number of dental clinics; and the rising awareness regarding oral healthcare are supporting the high growth of the oral care market in the Asia Pacific region.

Approach 1: Bottom-up approach (By Product, By Country)

  • The size of the oral care market was arrived at by obtaining sales data for each oral care product segment (Toothpastes, Toothbrushes & Accessories, Mouthwashes/Rinses, Dental Accessories/Ancillaries, Denture Products, Denture Cleaning Products) in key countries (China, India, Germany, UK & US) through various secondary (paid and public) databases.
  • Thus, segmental product revenues, by country were estimated and validated by primary participants.
  • Further, the split share of each key country in the oral care market was estimated and extrapolated to arrive at the global oral care market size.

Approach 2: Company Presentations

  • The size of the oral care market was obtained from annual presentations of leading players
  • Share of oral care products in the overall health care/personal care market was obtained from secondary data and validated by primary participants to arrive at the total oral care market
  • The numbers were further validated from primary participants

Approach 4: Primary Interviews

  • As a part of the primary research process, an individual respondent’s insights on the market size and market growth were taken during the interview (regional and global, as applicable)
  • All the responses were collated, and a weighted average was taken to derive a probabilistic estimate of the market size and growth rate
