The high prevalence of preterm births across the globe is a major factor supporting the growth of this segment.

By application, the oncology segment to accounted for the largest share of the market

The oncology segment accounted for the largest share of the global market. The increase in the prevalence of various types of cancers, especially head and neck cancers, cancers of the gastrointestinal system, lung cancer, and liver cancer, is expected to play a major role in the higher adoption of enteral feeding devices used to provide clinical nutrition to cancer patients.

By end user, the hospitals segment accounted for the largest share of the market

The hospitals segment accounted for the largest share of the market. The development of technologically advanced enteral feeding devices and the shift from parenteral nutrition to enteral nutrition are supporting the growth of the hospitals segment.

North America to account for the largest market size during the forecast period.

North America is expected to hold the largest market size in the market during the forecast period, followed by the European region. The highest market share of North America can be attributed to the high adoption of enteral feeding devices among the aging population, increasing prevalence of chronic diseases, the presence of many large hospitals, a shift from parenteral to enteral feeding, and the development of innovative products. The presence of a large number of global players in this region is another key factor contributing to the large share of this market segment. On the other hand, the Asia Pacific market is expected to witness the highest growth during the forecast period, primarily due to the rising preterm births, rising aging population, growing awareness of enteral nutrition, and unfulfilled medical needs in emerging economies with the rapid development of healthcare facilities.

Market Dynamics

Driver: Shift from parenteral to enteral nutrition

Enteral nutrition is more physiological, with fewer complications as compared to parenteral nutrition. The major advantage of enteral nutrition is the maintenance of the GI mucosal barrier, stimulating intestinal blood flow. Enteral administration also helps in maintaining small intestine mass and pancreatic functions, improves healing & recovery, reduces muscle catabolism, and lowers the risk of infection and contamination. According to an article published in the German Medical Science journal, major septic complications affect only 3% of patients on enteral nutrition as compared to over 20% of those on parenteral nutrition.

Technological advancements have also led to the development of compact and portable enteral feeding pumps, which are constructed using materials that prevent cracking in high-stress applications. Advanced features in pumps include appropriate programming options, programmed flushing intervals, multiple language options, occlusion pressure alarm options, history of previous feed rates, and screen and program lock-out features to prevent manipulation. Enhancements in pump designs (according to the requirements of healthcare providers and patients) and growth in the home healthcare market (enabling enteral feeding of nutrients to patients at home) are expected to propel the growth of the enteral feeding pumps market. Furthermore, the cost of enteral nutrition is lower than parenteral nutrition. The average cost of enteral nutrition per patient per day is about 40% of that of parenteral nutrition. These benefits have led to a significant increase in the adoption of enteral feeding in various treatment regimes.

Restraint: Complications associated with the enteral feeding devices

Enteral feeding is an efficient and cost-effective option for providing clinical nutrition to patients. However, there are a few complications associated with the use of these devices, which result in additional healthcare costs. Most of the complications associated with enteral feeding are minor, but some can be quite serious.

Complications can be categorized into three main sections:

  • Problems encountered during the placement of feeding tubes and delivery of enteral formula
  • Clinical and metabolic problems
  • Nutritional problems

Metabolic complications of EN are very similar to those occurring during parenteral nutrition, despite their lower incidence and severity. Careful monitoring can help reduce or prevent these problems. However, these concerns, coupled with the mortality associated with these complications, act as a restraint to the greater adoption of enteral feeding devices.

Opportunity: High growth opportunities in emerging countries

The Asia Pacific region is expected to offer potential growth opportunities to market players in the coming years. This can primarily be attributed to the diversified healthcare markets in this region, growing incidence of infectious and chronic disorders, and increasing R&D initiatives to develop innovative medical technologies. In addition, infrastructural developments in healthcare facilities, increasing healthcare expenditure, and the low-cost manufacturing advantage offered by emerging APAC countries are expected to encourage market players to invest in this region in the coming years.

Cancer is one of the most prevalent diseases in developing countries such as India, China, and Brazil. According to the Indian Council of Medical Research (ICMR), the total number of new cancer cases in India in 2018 was 1.16 million, which is likely to reach 1.73 million by 2020. Enteral feeding is considered the gold standard for the treatment of malnutrition in cancer patients. Thus, owing to the increasing prevalence of cancer in India and China, the demand for enteral feeding devices in these countries is expected to grow.

Apart from this, the largest number of preterm births are registered in Asia. According to the WHO (2018), 60% of preterm births occur in Africa and South Asia, with India, China, Pakistan, and Indonesia accounting for the largest number of preterm births. Improvements in the healthcare systems in these countries, coupled with the rising awareness about treatment options among patients, have resulted in an increase in the adoption of advanced technologies (including enteral feeding devices) in these countries. Furthermore, the increase in the per capita income and healthcare expenses in developing markets across APAC and Latin America has increased patient access to advanced healthcare treatments. However, some countries in the APAC and Latin America rely heavily on the purchase of medical devices (including enteral feeding devices) from developed countries such as the US, Japan, Germany, and Ireland.

Challenge: Insufficient reimbursement for enteral nutrition

Enteral nutrition therapy is reimbursed in developed countries such as the US, Canada, Germany, France, the UK, and Japan. However, reimbursements for enteral feeding in developing countries across the Asia Pacific, Latin America, and the Middle East are insufficient. This poses a significant challenge to the widespread adoption of enteral feeding in developing countries. Furthermore, decreasing federal funding and subsequent reimbursement cuts for home healthcare services over the next five years are expected to add to the challenges faced by players in the enteral feeding devices market.

Reimbursement facilities for enteral nutrition are made available in the US under Part B of the Medicare program. However, this reimbursement service is not applicable to any services furnished by non-physician professionals (home care settings). According to the MDPI article (2017), financial support for home enteral nutrition (HEN) is only available in 40% of countries, globally. In 2016, the CMS cut payments by USD 260 million for home healthcare agencies. In July 2017, CMS proposed a further 0.4% decrease, USD 80 million for providers in 2018 and USD 950 million in 2019. This is a significant concern in the market as non-certified home care agencies (60%) will face this issue of reimbursement cuts. Currently, the healthcare system in developing countries is facing a number of challenges owing to the rising healthcare costs, increasing incidence of chronic diseases, and growing geriatric population (coupled with the growth in age-related disorders). In order to counter these challenges, governments in a number of countries are focusing on redesigning their healthcare reimbursement systems. In Asian countries, there is very limited or no reimbursement available (except in Japan). Thus, limited or decreasing reimbursement rates in developed countries and undefined reimbursement policies in emerging countries are restricting the adoption of medical devices and, in turn, challenging the growth of the market globally.
